- W2018673575 abstract "<i>Objectives:</i> Uvulopalatopharyngoplasty (UPPP) has been performed worldwide for nearly two decades. Despite the relatively large accumulated experience, the indications for its performance, the optimal surgical technique and its success rate are all still unclear. Patients with sleep-related breathing disorders have an abnormal velolinguopharyngeal structure. Indeed, some of them are also symptomatic when they are awake, breathy oral respiration and nasal obstruction being their most common manifestations. The compromised upper airway configuration in wakefulness predisposes them toward development of sleep-related breathing abnormalities. The objective of this article is to clarify the role of UPPP in improving upper airway aerodynamics. <i>Methods:</i> Clinical evidence, intraoral photographs and lateral radiographs are presented to illustrate the role of UPPP in the improvement of upper airway aerodynamics. The anatomic characteristics of patients having disordered upper airway dynamics, indications of performing UPPP and its optimal technique for improving upper airway dynamics during sleep and wakefulness will be presented and discussed along with the relevant literature. <i>Conclusions:</i> UPPP should be considered a method of improvement of upper airway dynamics in selected patients suffering from breathing difficulties including nasal and oral obstruction. Candidates for UPPP should be selected relying on otolaryngologic examination including flexible nasopharyngoscopy, on cephalometry and on polysomnography. UPPP effectiveness depends on rational selection of candidates. Adverse effects and complications can be diminished by a good operative technique." @default.
