- W2019133072 abstract "RationaleAdolescents with asthma are vulnerable to the consequences of risk behaviors because of their chronic condition and medications. Substance use, a common occurrence for this developmental stage, requires early identification of those most medically at-risk.MethodsA correlational study with 68 teens with asthma was conducted at two university medical centers. This analysis explored factors associated with poor decision making and substance use (smoking, alcohol use, drug use) among adolescents with asthma. Disease and treatment factors were extracted from the medical record and/or parent and teen self-reports. Abstract ability (weighing current/future consequences) and psychosocial outcomes were assessed by measures administered in a semi-structured interview and with the teens using online data entry. Negative-binomial regression was used to predict poor decision making; logistic regression was used to predict lifetime and current substance use.ResultsPoor decision making was predicted by younger age (p=0.034), more school problems (p=0.001), and less symptomatic asthma (p=0.031). Lifetime substance use was positively associated with abstract ability (p=0.021), poor decision making (p=0.034), and more risk motivation (p=0.004). Age was marginally associated with lifetime substance use (p=0.060), with older teens more prone to using substances during their lifetime. Current substance use was predicted by older age and more risk motivation.ConclusionsHigh abstract ability and school problems in young asthmatic teens present challenges requiring further neurocognitive evaluation and intervention to prevent substance use. Many teens with asthma are taking unacceptable risks with their health and further study of a clinical profile may help identify those in need of personalized decision intervention.
