- W2019473772 abstract "56才女性．乳癌と卵巣癌を6年間の間隔を経て発症し，各々に関連して皮膚筋炎を合併した．51才時，乳癌切除の半年後，全身に定型的皮疹が出現し，筋電図上も筋炎の所見が得られたため，皮膚筋炎と診断された．皮疹はステロイド外用薬のみで2ヶ月程度で軽快し，以後再発なく経過していた．しかし6年後再び同様の皮疹，Gottron徴候，筋原性酵素の上昇がみられ，皮疹・皮膚生検所見と合わせて皮膚筋炎と診断した．悪性腫瘍に対する精査を行ったところ卵巣癌が発見され，腫瘍切除が行われた．腫瘍切除後，ステロイドの外用のみで皮膚症状・筋原性酵素ともに軽快し，また乳癌の再発所見もみられなかったため，卵巣癌に伴う皮膚筋炎と考えられた．2回とも悪性腫瘍診断の前後に皮膚筋炎を発症しており，また2回目は腫瘍切除後，皮膚筋炎症状の軽快をみたため，それぞれ腫瘍随伴症候群としての皮膚筋炎発症が強く示唆された．異なる臓器・組織型の悪性腫瘍にそれぞれ皮膚筋炎を合併した症例は検索した限りではみられず，稀な症例であるとともに，悪性腫瘍に伴う皮膚筋炎の発症機序について腫瘍の種類に関わらず起こる腫瘍随伴症候群と捉える上で，示唆に富む一例と考えられた．" @default.
