- W2019804490 abstract "To report the incidence of acute and chronic rectal bleeding, based on the patient reported Expanded Prostate Cancer Index Composite (EPIC) score, and to investigate factors predictive of rectal bleeding in patients who underwent prostate brachytherapy (PB) with Cesium-131 (131Cs). EPIC surveys in 219 patients who underwent PB with 131Cs seeds were analyzed. Preoperative and postoperative EPIC scores were obtained at 2 and 4 weeks, and 3, 6, 9, 12, 18, 24, 30, 36 months. For each time point, the incidence and frequency of rectal bleeding as well as how problematic it was considered by patients were recorded. Acute was defined as bleeding occurring within the first 3 months postoperatively, while chronic was defined as bleeding occurring any time from 6 months to 3 yrs as reported by EPIC questionnaire. If patients reported bleeding as anything more than “rarely,” this was recorded as an event for analysis. Patients also rated how much of a problem rectal bleeding was to them at each time point. Various factors were also evaluated for correlation with rectal bleeding such as age, diabetes, tobacco use, total number of seeds, total number of needles, number of central and peripheral seeds, number of central and peripheral needles, seed activity, baseline prostate volume, and the use of hormone therapy and/or external beam radiation therapy. Preoperatively, 0.9% reported rectal bleeding, and 50% of these patients considered it a very small problem, and 50% considered it a moderate problem. Acute rectal bleeding was reported by 16.4%. At 3 months, 80.6% of these patients did not consider rectal bleeding a problem, 8.3% considered it a very small problem, 2.8% a small problem, 8.3% a moderate problem, and no patient considered it a big problem. Chronic bleeding was seen in 18.7%. Of these patients, 27.5% did not consider it a problem at all, 20% a very small problem, 7.5% a small problem, and 2.5% a big problem at 36 months. Per univariate analysis, combined modality therapy with external beam radiation predicted for acute and chronic bleeding. In addition, acute bleeding predicted for chronic bleeding. To our knowledge, this is the first report on patient reported rectal bleeding in prostate brachytherapy. The incidence appears higher than that seen in the literature based on physician reports. Only a minority (2.5%) felt it to be a big problem at 36 months. There was a significant correlation between the use of combined modality therapy with incidence of acute and chronic significant rectal bleeding.
