- W2019922896 abstract "It was reported that cardiopulmonary bypass (CPB) deprived metaboliam in the lung, and that pulmonary circulation before the end of CPB could restore increase in prostaglandin E2 (PGE2), systemic tissue perfusion injury and body oxygen metabolism deterioration. However, in lung transplantation (Lx), pulmonary circulation is restored on graft, which may induce another CPB reaction. We prospectively examined influence of CPB on body oxygen metabolism in Lx. On 10 dogs (experimental species), left Lx was successfully performed (Group-On: with normothermic CPB without cardiac arrest, Group-Off: without CPB; n=5 vs. 5). Lx was started at 30 min. of CPB, the graft was re-perfused for 10 min. before the end of CPB, and CPB duration was controlled for 120 min. At 30 min. after CPB, right pulmonary artery and bronchus were clamped. Also in Group-Off, the time-schedule was almost equal. Body weight, donor per recipient body weight ratio and clinical parameters were comparable between the two groups except hematocrit level during CPB. At 90 min. after graft perfusion (CPB), SvO2 was lower (P<0.01), O2 extraction rate was higher (P<0.01) and PGE2 was higher (P<0.01) in Group-On than in Group-Off while those were comparable before graft perfusion between the two groups. O2 consumption was higher in Group-On than in Group-Off whereas O2 delivery was comparable between the groups. In conclusion, Lx during CPB may induce the new inflammatory reaction and influence body oxygen metabolism contrary to pulmonary circulation." @default.
