• W2019934937 abstract "To estimate if cervical length (CL) measured by transvaginal ultrasonography (TVUS) predicts spontaneous preterm birth (SPTB) < 35 weeks in women with a history of SPTB stratified by SPTB history subtype (PPROM or PTL with intact membranes at labor onset). Women with a history of SPTB who subsequently became pregnant with singleton gestations were compared with a low-risk control group. TVUS CLs were performed at 24–30 weeks. Primary outcomes were CL and SPTB < 35 weeks. Secondary outcomes included funneling, SPTB < 37, < 34, and< 32 weeks, low birth weight, maternal and neonatal outcomes. Sample size was based on detecting a 5 mm difference in CL and a difference in SPTB from 2% (low risk) to 20% (history of SPTB) with α = 0.05 and β = 0.20. Multiple logistic regression (MLR) was used to control for confounders and calculate OR and 95% CI. ROC curves were used to determine the best cut-off for TVUS to predict SPTB < 35 weeks. Women with a history of SPTB with PTL (n = 42) had a shorter CL (3.28 cm) than women with a history of SPTB and PPROM (n = 48, 3.77 cm, P = 0.021), and both subgroups had shorter CLs than the low-risk control group (n = 103, 4.30 cm, P < 0.0001). Both subgroups were associated with SPTB < 35 weeks, < 37 weeks, < 34 weeks and birth weight < 2500 g. ROC curves determined the best cut-off of CL to predict SPTB < 35 weeks and < 37 weeks to be < 3.0 cm. By MLR, the only independent predictors of SPTB < 35 weeks were CL < 3.0 cm, history of SPTB and antepartum hemorrhage in the current pregnancy. In women with a history of SPTB, using a cut-off of 3.0 cm, TVUS had PPV = 28.0% and NPV = 93.8% for SPTB < 35 weeks, and PPV = 44.0% and NPV = 84.6% for SPTB < 37 weeks. Women with a history of SPTB with PTL and intact membranes at labor onset have shorter CLs than women with a history of SPTB and PPROM, and both groups have shorter CLs than a low-risk control group. Both groups of women with a history of SPTB have an increased risk of SPTB < 35 weeks and this is predicted by a TVUS CL < 3.0 cm." @default.
