We therefore considered the questions: in what way does a mediocranial displacement of the hip joint center alter 1. the magnitude and 2. the direction of the resultant force? Biomechanical tests were carried out on a human multibody computer model. After displacement of the hip joint center, the resultant forces were calculated for the flexed single leg stance. We considered also the question: to which extent was the hip joint center displaced in total hip arthroplasty in rheumatoid patients? A decrease of the hip joint force was observed with a medial displacement, an increase with a cranial displacement. A mediocranial displacement of up to 10 mm in both directions reduces the load from 2595 N of the original position to 2142 N. The results presented here suggest a self-limitating effect as the thin and osteopene acetabular bone may be protected by a lower hip force in Protrusio acetabuli. A full cranial and posterior acetabular reconstruction in total hip arthroplasty will likely decrease hip load. With regard to the direction of the resultant hip force, a rather low model sensitivity was seen with angle changes below 2.5 °. The results of pelvic X-ray morphometry showed a Protrusio acetabuli in 14 of 31 rheumatoid patients (45.2 %). An average joint center relocation after total hip arthroplasty of 7.2 mm in the lateral and 5.7 mm in the distal direction was measured." @default.
