- W2020498749 abstract "To evaluate the outcome of monochorionic (MC) twins with selective growth restriction (sIUGR) hospitalized under strict surveillance. We retrospectively evaluated all MC twin pregnancies hospitalized at a single tertiary center between 08/2005 and 08/2008 (study group, 30 pregnancies). IUGR was defined as birthweight below 10th percentile matched for twins and fetal gender. Patients with sIUGR were hospitalized in the high-risk unit and monitored by NST (TID), BPP (QD) and Doppler (every other day). The control group (79 pregnancies) included uncomplicated MC pregnancies, all with birthweight > 10th percentile. 28 fetuses had a birthweight below 10th percentile and 2 below 1st percentile). The patients were hospitalized for 20 ± 18 days in the high-risk unit.13 fetuses with absence of diastolic velocity flow in the umbilical artery were hospitalized for 28 ± 19 days (p = 0.01 compared to normal flow). None of the hospitalized patients had intra-uterine fetal death. Patients with sIUGR were delivered significantly earlier than controls (33.2 vs. 35.2 weeks, p < 0.01). The mean birthweight of the smaller fetus (1403g ± 380 vs. 2166g ± 440) and larger fetus (1897g ± 440 vs. 2366g ± 450) were significantly smaller in the sIUGR group (p < 0.001). Mean discordancy in the sIUGR group was significantly higher than controls (26.2 ± 9.0% vs. 8.3 ± 8.0%, p < 0.001). Apgar scores at 1- (8.4 ± 1.3 vs. 8.7 ± 0.9) and 5-minutes (9.4 ± 1.1 vs. 9.8 ± 1.0) were not significantly different between the sIUGR and controls nor between the sIUGR with normal flow compared to AEDV. 2/60 fetuses in the IUGR group and 1/156 fetuses in the control group were delivered with 5-minutes Apgar score < 7 (p = 0.18). The neonates (study group) were hospitalized in the NICU for 31 ± 18 days. Neonatal mortality was 1/60. 5/59 neonates had abnormal cranial US before discharge. Patients with sIUGR in MC pregnancies hospitalized under strict surveillance have relatively good short term outcome. Long term outcome is still to be assessed in long term studies." @default.
