• W2020528051 abstract "Objective: To determine the neuroanatomic basis for impaired self-awareness of functional abilities in neurodegenerative patients. Background Neurodegenerative patients, particularly those with right-brain damage, present with poor awareness of their functional limitations that may exacerbate their already jeopardized decision-making and worsen behavior. However, structural anatomic correlates of this deficit remain unclear. Design/Methods: 124 participants (27 Alzheimer9s disease, 17 behavioral variant frontotemporal dementia [bvFTD], 6 predominantly right temporal FTD [rtFTD], 6 semantic variant primary progressive aphasia [svPPA], 5 non-fluent aphasia [nfPPA], 10 progressive supranuclear palsy, 6 amyotrophic lateral sclerosis and 47 healthy older adults) described themselves on the Personal Competency Rating Scale (PCRS), assessing observable functioning across four domains (daily living activities, cognitive, emotional, interpersonal), and underwent structural MRI. Informants also described subjects on the PCRS, yielding discrepancy scores (self minus informant ratings: 0 =accurate evaluation; positive scores=overestimation; negative scores=underestimation). Discrepancy scores group effects were analyzed using GLMs, controlling for age, sex, and disease severity (MMSE). Discrepancy scores were correlated with brain volume using DARTEL-based voxel-based morphometry analysis of combined gray/white matter maps. To identify their unique neural correlates, overestimation and underestimation scores were analyzed separately, controlling for age, sex, MMSE, and total intracranial volume. Results: bvFTDs overestimated their functioning in all domains, nfPPAs overestimated emotional functioning, and rtFTDs overestimated interpersonal functioning (p Conclusions: Though patients with typical bvFTD significantly overestimate their functioning across various domains, those with predominantly temporal (rtFTD) anatomy show more preserved self-awareness of functioning, except for their social behavior. Our finding that poor self-awareness correlates with prefrontal and right anterior insular atrophy may explain this dissociation in insight between bvFTD subtypes.
