This text is the first volume in a series entitled “Atlas of Operative Surgery,” which intends to cover the basic components of general surgery. The purpose of this book is to provide thorough, lucid descriptions of standard peripheral vascular operations for surgeons-in-training and general surgeons. The authors have intentionally excluded references and discussions of pathophysiology, diagnosis, perioperative care, and results. Operative indications and potential complications are occasionally addressed in an abbreviated fashion. The conduct of each procedure is explained in the style of a detailed operative report, including such elements as the name and size of vascular clamps or suture material to be used. Abundant line drawings complement the text in every step of the operation. Anatomic relationships and operative exposure are appropriately emphasized and clearly demonstrated. A broad spectrum of vascular operations follows an introductory section on basic technique. Extracranial cerebral vascular, aortic arch, upper extremity, thoracic and abdominal aortic, visceral and infrainguinal reconstructions are equally exhaustively described. Also included are sections on fasciotomy, amputations, sympathectomy, thoracic outlet compression, superior and inferior vena cava, and venous insufficiency. Although the authors strived to describe standard techniques, there are certain to be differences of opinion regarding minor technical points. For example, the authors' preferences for albumin-autoclaved aortic grafts, fibulectomy-fasciotomy, and the lateral exposure of the peroneal artery for in situ bypass may not be shared by all experts. Although the descriptions of lower extremity bypasses are among the best available, no mention is made of alternative autologous veins such as the cephalic vein. Venous reconstructive procedures are considered “in evolution” and only a brief description of one of these techniques, vein valve transposition, is available in this section. In addition, presentations on uncommon entities such as carotid body tumors and visceral artery aneurysms are included, but no mention is given to vascular access surgery—procedures more likely to be performed by the intended readers of this manual. Despite minor limitations, this is an excellent guide to the surgeon-in-training and is recommended to all general surgery residents and vascular fellows as a guide to the smooth performance of peripheral vascular procedures. Experienced general surgeons will appreciate the detailed analysis of operative techniques by these respected authors and will find this a valuable resource." @default.
