METHODS We retrospectively reviewed the medical records of 2036 patients who underwent radical prostatectomy for prostate cancer between 1994 and 2011. 233 patients without neoadjuvant therapy were eligible for active surveillance based on Epstein criteria defined as clinically T1 disease, prostate-specific antigen (PSA) density ≤ 0.15ng/mL2, Gleason score ≤ 6, fewer than three cores containing prostate cancer, and up to 50% of cancer involvement in any core. Clinically significant prostate cancer was defined as pathologic Gleason sum ≥ 7 or non-organ confined disease (extracapsular extension or seminal vesicle invasion). We investigated pathologic outcomes and predictive factors for clinically significant prostate cancer. RESULTS Of total 233 cases, patients with pathologic Gleason sum ≥ 7 were 91 (42.1%), extracapsular extension were 11 (4.7%), and seminal vesicle invasion were 3 (1.3%). Patients with clinically significant prostate cancer were 42.1% (98/233). When comparing clinically insignificant and significant prostate cancer, there were significant differences in mean age (63.7 vs 65.9, p=0.007), prostate volume (47.1ml vs 42.2ml, p=0.021), PSA density (0.096ng/ml2 vs 0.104 ng/ml2, p=0.03), maximum tumor volume in biopsy core (11.6% vs 18.3%, p<0.001), rate of 2 positive cores (19.3% vs 39.8%, p=0.001). On multivariate analysis, mean age (p=0.022), PSA density (p=0.017), maximum tumor volume (p=0.001) and 2 positive cores (p=0.005) were independent predictive factors for clinically significant prostate cancer. CONCLUSIONS Significant proportion of patients who are candidates for active surveillance had clinically significant prostate cancer. Especially age, PSA density, maximum tumor volume and 2 positive cores were independent significant predictive factors for clinically significant prostate cancer.
