FeinendegenKnowledge about the role of the thalamus in higher cortical function is limited.Isolated thalamic lesions were hard to document before the era of brain-imagingmethods (CF and MRI) and only a few cases with pathological verification exist(Hogousslavsky et al., 1988). However, analyses of neurological and neuro-psychological deficits that rely on structural abnormalities rather than on metabolicstudies may be misleading, as evidenced by the present communication . We studied atotal of four patients with brain-imaging techniques (CT, MRI, and PET), detailedneuropsychological examination, and clinical examination for thalamic infarctionof different locations. The following case of a patient with a left paramedian territorystroke is representative.Case Report. A 53-year-old right-handed man with several risk factors ofatherosclerosis was admitted to the hospital. The patient was initially somnolent anddysarthric, but was oriented in time and space. In addition, a mild right-sidedpreferentially proximal hemiparesis with impaired bilateral antagonistic andsynergistic movements (Freund and Hummelsheim, 1985) and right-sidedhemihypcsthesia, particularly of the upper part of the body, for all modalities werepresent. A vertical gaze paralysis with reduced optokinctic nystagmus and skewdeviation with the left eye in a lower inward-rotated position and the right eyeelevated in an outward position as well as an internal left ophthalmoplegia recoveredslowly and incompletely during followup. Although he could repeat a given rhythmwith either hand, he was unable to produce it with both hands. A severe, primarilyanterograde but also retrograde memory disturbance was the most impressivepersistent deficit. No deficits were found on six screening tests for visual, auditory,tactile, kinesthetic, motor, and conceptual neglect (Halsband et al., 1985).Neuropsychological examination showed a striking discrepancy between memorystorage and memory retrieval: Under immediate and delayed conditions, he was ableto remember visual, tactile, auditory, motor, and verbal information if these werepresented again among others; however, he was severely impaired when asked torecall some of the information without repeat presentation .
