- W2021691796 abstract "Standardized procedures to measure neuroimaging biomarkers are necessary to better understand the neurodegenerative process of Alzheimer 's disease (AD). Several worldwide initiatives have been developed, the first one of which is the United States Alzheimer's Disease Neuroimaging Initiative (US- ADNI) which has established a platform to track the brain changes of Alzheimer's disease using standardized procedures. Patients and controls were enrolled in 9 Italian memory clinics between January 2007 and December 2011.Overall 395 consecutive patients were enrolled with subjective memory impairment (SMI, n = 10), mild cognitive impairment (MCI, n = 100), Alzheimer's Dementia (n = 248), and fronto-temporal dementia (n = 13), as well as 24 cognitively normal elders (CNE). All subjects underwent clinical and neuropsychological assessment highly consistent to those used in the US-ADNI to harmonize clinical diagnoses across centres. All centres underwent scan harmonization using US-ADNI phantom and then acquired magnetic resonance (MR) images compliant with US-ADNI MPRAGE protocol for 1.5T or 3T scans. All structural MR images were assessed using US-ADNI quality controls procedures. One human tracer per memory clinic was trained to manually segment the hippocampus on 10 US-ADNI scans of variable atrophy severity according to a standardized manual hippocampal segmentation protocol. Then, these tracers segmented the hippocampi from their own MR images. A discriminant analysis was carried out in AD and CNE subjects to test the percentage of cases correctly classified by the manual hippocampal protocol and by the Freesurfer automatic segmentation. Patients with MCI demonstrate greater memory impairment than SMI and CTRL individuals, but the former are not as impaired as the subjects with AD. Hippocampal tracing test-retest reliability was greater than 0.80 for each tracer. Inter-rater reliability figures among memory clinics were 0.96 and 0.92 for 1.5T and 3T MRI respectively. Ninety-one percent of subjects (137/147 AD, 14/19 CNE) were correctly classified by the manual segmentation while 82% of subjects (129/147 AD, 15/19 CNE) were correctly classified by automatic segmentation. Standardized procedures such as those of ADNI are critical instruments for memory clinics to use Alzheimer's Disease biomarkers by minimizing error variance due to across-site variability and maximizing sensitivity to true biological signal." @default.
