- W2022125746 abstract "Introducción y objetivos: MammaPrint® es una micromatriz que clasifica los pacientes con cáncer de mama en 2 grupos (mal y buen pronóstico) según el riesgo de desarrollar metástasis y estima el beneficio de la quimioterapia en cada paciente. Nuestro objetivo fue comprobar la validez clínica y analítica de MammaPrint® para determinar el riesgo individual de metastatización del cáncer de mama. Material y métodos: Realizamos una revisión sistemática de la literatura médica y consultamos MedLine, Centre for Reviews and Dissemination Database, ECRI Institute, Cochrane Library y el metabuscador Tripdatabase. Se incluyeron mujeres con carcinoma de mama en las que se empleara Mamaprint® frente a otros sistemas pronóstico o técnicas moleculares. Se midieron la reproducibilidad y la validez de la prueba, así como los resultados clínicos en términos de la capacidad de predecir la mortalidad y el tiempo libre de metástasis. Resultados: Se seleccionaron 10 estudios de los 75 documentos localizados. Un único trabajo evaluó la fiabilidad y la validez analítica de MammaPrint®. En cuanto al riesgo de metástasis asociado a la firma de mal pronóstico frente a buen pronóstico, este osciló entre 5,7 (intervalo de confianza [IC] del 95%: 1,6–20) para un seguimiento de 5,8 años y 2,32 (IC de 95%: 1,35–4,00) para un seguimiento de 13,6 años. MammaPrint® se identificó como un factor pronóstico de recurrencia independiente (odds ratio: 4,6 [IC del 95%: 2,3–9,2]). Conclusiones: MammaPrint® podría aportar beneficios en la selección del tratamiento más adecuado en el cáncer de mama, aunque las conclusiones no evidencian su utilidad clínica en pacientes en los que se augura a priori un beneficio de la aplicación de quimioterapia. MammaPrint® is a micromatrix based test and the result classifies analyzed tumors as low or high risk for recurrence of breast carcinoma. By analyzing the individual activity of these genes, MammaPrint® estimates the response to chemotherapy and/or predicts the outcome of the breast neoplasia. We aimed to assess the clinical and analytical validity of MammaPrint® that determines individual risk of relapse of breast cancer. Systematic review of the literature using a structured database search (MedLine, CRD, ECRI Institute, Cochrane Library) and Tripdatabase metasearch. The criteria for selecting report for inclusion were the following: population (breast carcinoma), intervention (MammaPrint® versus medical outcome systems and molecular techniques), and analytic (reproducibility and validity of the method) and/or clinical (the ability to predict mortality among patients, metastasis-free time) results. We found 75 documents. After eliminating the studies that did not satisfied the inclusion criteria, 10 studies were evaluated. Only one paper analyzed MammaPrint® reproducibility, indicating reliability and validity of this methodology. The risk of distant metastasis associated with poor prognosis signature versus good prognosis, ranged from 5.7 (IC95%=1.6–20) for 5.8 year follow-up to 2.32 (IC95%=1.35–4.00) for 13.6 year follow up. MammaPrint® was identified as an independent predictor of risk for metastasis, as well as the tumor diameter and non-use of chemotherapy. MammaPrint® test could bring benefits to the diagnosis and treatment of breast cancer patients but there was no evidence to conclude its clinical usefulness in patients who predicts an a priori advantage to chemotherapy." @default.
