Background. Patients referred to home hospice often develop pain, dypsnea, delirium, and other problems as they near end of life. Hospice emergency kits (HEKs) contain medications aimed to enable rapid response and relief of emergent symptoms during the dying process. Research objectives. Understand the benefits and burdens of a HEK. Methods. We performed a retrospective chart review of deceased veterans from Birmingham VAMC who had been, prior to death, discharged with hospice. In addition, questionnaires were given to hospice nurses at sixteen community hospices that provide care for VA patients. Results. HEKs were distributed to 89% of patients referred to home hospice from the BVAMC in 2009. Use of the HEK was documented in 50%. Documentation of the use of each item was as follows: • total patients dispensed HEK N = 68 • patients utilizing HEK N = 34 • morphine concentrate N = 25 • antibiotic N = 12 • haloperidol N = 10 • furosemide N = 7 • lorazepam N = 5 • scopalamine patch N = 4 • promethazine N = 0. In 22%, there was documentation suggesting that the HEK-facilitated death at home versus hospital. For one patient there was concern of inappropriate use of medications. Anonymous questionnaires were returned by 78 hospice nurses (49% of those surveyed). Nurses estimated that when HEKs were available they were used 65% of the time. Fifty-five percent of nurses felt HEKs were helpful 100% of the time; 79% reported it was helpful at least 70% of the time. The vast majority (86%) believed HEKs reduced caregiver/patient anxiety. Symptoms reported as most commonly addressed were agitation, shortness of breath, pain, and secretions. Conclusion. HEKs appear to be a safe, effective, and inexpensive method of improving patient comfort, family confidence, and nurse effectiveness in the management of home hospice patients. Implications for research, policy, or practice. It is likely that HEKs increase comfort and frequency of dying at home. More research is needed to define the ideal HEK and improve its implementation." @default.
