- W2022185251 abstract "To compare operative time and intra- and post-operative complications between laparoscopic (TLH) and robotic-assisted total laparoscopic hysterectomy (RA-TLH). Blinded, randomized controlled trial. (Clinicaltrials.gov #NCT00485355). Two academic institutions. Participating surgeons were experienced in TLH and RA-TLH. Subjects consisted of women who planned laparoscopic hysterectomy for benign indications. Subjects were excluded for suspected malignancy, medical illness precluding laparoscopy, morbid obesity, or need for concomitant bowel resection. A priori, it was determined that 23 subjects in each arm were needed to detect a difference of 30 minutes or greater in operating time between TLH versus RA-TLH with 90% power and a significance level of .05. 57 women consented, enrolled, and were randomized while 53 women underwent surgery (TLH n = 27, RA-TLH n = 26). Pre-operative randomization to TLH or RA-TLH was stratified by uterine size (> or ≤12 weeks) and subjects were blinded to randomization through the 6 week study period. Additional procedures were performed at the surgeon's discretion. Mean subject age and body mass index (± SD) were 44.9 ± 6.2 years and 30.7 ± 7.4 with no difference between groups. Additionally, there were no differences in other demographic variables. Compared to TLH, operative time (skin incision to skin closure) was significantly longer in the RA-TLH group (mean difference +77 minutes [95% CI 33–121, p<0.001] as was total surgical time (entry into OR to exit) (mean difference +72 [95% CI 14–130, p = 0.016] minutes). Mean docking time was 6 (± 4) minutes. There were no significant differences between groups in estimated blood loss, pre- and post-operative hematocrit change and length of stay. There were very few complications, with no difference in individual complication types or total complications between groups. Although laparoscopic and robotic-assisted hysterectomy are safe approaches to hysterectomy, robotic-assisted hysterectomy takes significantly longer." @default.
