- W2022237689 abstract "RATIONALE: Subepithelial fibrosis often develops in eosinophilic esophagitis (EE). This fibrosis may lead to dysphagia, food impaction and esophageal strictures. Often endoscopic biopsies are not adequate to assess subepithelial fibrosis, particularly in young children. We retrospectively examined the relationship between esophageal subepithelial fibrosis, basal cell hyperplasia (BCH) and eosinophil count before and after treatment (swallowed inhaled corticosteroids and/or selective elimination diet) of pediatric EE patients. METHODS: A review of subjects diagnosed with EE (using ≥20 eos/hpf) at a single tertiary pediatric facility from 2000-2006 was performed. H&E and trichrome stained proximal and distal esophageal biopsies at diagnosis (Pre-treatment), post-treatment (Post 1) and further into treatment (Post 2) were then evaluated by pathologists for eos/hpf, BCH, and subepithelial fibrosis (Chehade 2007). Thirty-one subjects with pre-treatment and post-treatment esophageal biopsies were included. Spearman rank correlations for eos/hpf with BCH and fibrosis were calculated. RESULTS: For each subject, there was a positive correlation between fibrosis score and BCH within each time point: Pre-treatment (n=31) R=0.603, p<0.001; Post 1 (n=26) R=0.667, p<0.001; Post 2 (n=25) R=0.773, p<0.001. We did not observe consistent relationships between eosinophil count and BCH or fibrosis. CONCLUSIONS: Basal cell hyperplasia and subepithelial fibrosis were present together and correlated positively within each time point. BCH may be a useful surrogate marker for fibrosis, particularly in patients in whom biopsies are not adequate to assess subepithelial fibrosis. Consistent relationships between eosinophil count and either BCH or subepithelial fibrosis were not demonstrated." @default.
