Fu, Chua and Chang [1] in the June 2008 issue of the Journal of Medical Ultrasound extensively reviewed the diagnosis of mitral regurgitation (MR) using echocardiography. This review article covers many aspects, including the quantitative assessment, the pathophysiologic mechanism and the timing and choice of surgical modality for MR. This is an excellent review on the use of echocardiography in the evaluation of MR. In this article, quantitative parameters of echocardiography for the determination of MR severity were provided, which included the chamber size of the left atrium and left ventricle, and the Doppler and color Doppler measurements of mitral flow. The authors emphasized that left ventricular function and the severity of MR were the most important determinants in the prognosis of patients with MR and for the type of surgical intervention. The usefulness of three-dimensional echocardiography was also presented in this review, mainly for the identification of the pathologic determination of detailed mitral lesions. Yet, three-dimensional echocardiography is not as widely accessible as twodimensional echocardiography and transesophageal echocardiography. It seems that surgical decisionmaking for MR is little affected, even with a lack of three-dimensional echocardiographic evaluation. There are two points which must be particularly emphasized in the adoption of echocardiography for MR evaluation. Firstly, the symptoms of patients are important, both for the severity of MR and the indication for surgery, as emphasized in the ACC/AHA 2006 guidelines [2]. Secondly, it has long been known that the echocardiographic setting is very important for accurate evaluation of regurgitation imaging. The severity of regurgitation is evaluated as proportional to the area of the regurgitation jet. Different Doppler gain settings would affect the imaging to a significant extent." @default.
