- W2022384239 abstract "OBJECTIVE: To determine the relationship of ovarian morphology to the degrees of menstrual cycle dysfunction and insulin resistance in PCOS. DESIGN: Cross-sectional analysis. MATERIALS AND METHODS: 539 PCOS and 181 eumenorrheic controls were studied. Menstrual bleeding intervals were assessed and a pelvic ultrasound was performed to determine ovarian volume and antral follicle count. Parameters of ovarian morphology were correlated with age, body mass index, waist to hip ratio (WHR), homeostasis model assessment to evaluate insulin resistance (HOMA-IR), fasting glucose (FG), fasting insulin (FI), free testosterone, total testosterone (TT), DHEAS, prolactin (PRL), and thyroid stimulating hormone (TSH) values. RESULTS: Women with the most severe menstrual dysfunction (i.e. intervals ≥3 mos) have the most total antral follicles (mean 30.1 vs. 20.7, resp., p<0.05) and the largest total ovarian volumes (mean 23.3 cc vs. 13.5 cc, resp., p<0.05) compared to controls. No significant associations were found between HOMA-IR, FG, or FI, and the volume or antral follicle count of the left or right ovaries, total follicles, or overall volume. Multivariately, the diagnosis of PCOS and increasingly levels of TT and TSH were associated with more follicles overall, whereas increasing age and PRL levels were associated with fewer follicles; these factors accounted for 26.8% of the variation in follicle number. The diagnosis of PCOS, and increasing TT and WHR values, were associated with larger ovarian volumes, whereas higher PRL levels were associated with smaller ovarian volumes; these factors accounted for 15.8% of the variability in total ovarian volume. CONCLUSION: PCOS women with the most severe menstrual dysfunction had the highest total number of antral follicles and the largest total ovarian volumes. This relationship does not appear to be mediated by degree of insulin resistance, glucose intolerance, or hyperinsulinemia." @default.
