The white arrow indicates the distal esophagus.Duplications of the gastrointestinal tract are rare. They are thought to be developmental anomalies (1,2). Esophageal duplications are covered by serosa, contain smooth muscle, and are lined by gastrointestinal mucosa. They share a common blood supply with the gastrointestinal tract and are found on the mesenteric border when in the stomach and intestine. They most often occur in the ileum and colon, but can occur at any level of the gastrointestinal tract, including the esophagus in which they are found in the posterior mediastinum. Esophageal duplications are occasionally associated with vertebral anomalies, leading to speculation that a failure of separation of ectoderm and endoderm at the notochord stage of development prevents ventral fusion of the notochord. The esophageal cyst and vertebral anomalies then result from remnants of the yolk sac herniating between the halves of the notochord. Failure of recanalization of the esophagus may result in esophageal duplication without vertebral anomalies. The duplication is usually easily separated from the esophageal wall, although a shared wall may occur in some instances, making surgical removal more difficult. Most esophageal duplications cause respiratory symptoms of wheezing, obstruction, or aspiration. They rarely cause dysphagia and are not reported to cause abdominal pain. Some are found incidentally. The duplication cyst in this patient may have been an incidental finding unrelated to his chronic, recurring abdominal pain. Thoracoscopic removal, performed in this patient, is the treatment of choice for symptomatic and asymptomatic duplications of the esophagus (3,4)." @default.
