- W2022604538 abstract "Purpose: The prevalence of 3D volumetric dose based planning in brachytherapy of the breast and other anatomic locations has led to an increased interest in 3D dose planning for gynecologic brachytherapy. The purpose of this study is to quantify via 3D volumetric dose planning the dose reduction to adjacent structures that can be obtained via a multichannel approach relative to single channel approaches using a new flexible inflatable gynecologic applicator. Materials and Methods: In our study, twelve (12) clinical dose plans were generated from CT images of patients undergoing vaginal cuff brachytherapy with a new multichannel applicator (CAPRI device). Target volume was defined as a 5mm uniform expansion beyond the vaginal surface with localized contraction of the PTV at regions of encroachment of the bladder and rectum. The multi catheter array in the inflatable applicator allows for conformance to an irregularly shaped target volume. Target volume was recorded in each dose plan. Coverage was 95/95, dose was 700cGy to depth x 3 fractions. To assess the impact of multichannel dosing ability, 3D dose plans using available multi catheter radial arrays within the flexible applicator were compared with single channel source lumen plans with respect to max 2cc to the rectum and bladder, mean dose to the rectum and bladder, and mean dose to the mucosal surface. Results: When compared to single lumen dose plans, multi catheter plans with the CAPRI applicator provided max 2cc rectum and bladder dose reductions from 56.3% of surface dose to 50.6% (10.1% reduction) and from 64.0% to 57.4% (10.3% reduction), respectively. Mean dose to the rectum and bladder values were also reduced by 9.3% and 9.2%, respectively. Mean dose to the vaginal surface remained the same at 950 cGy/fx for single and multichannel plans. Mean target volume was 26.4 ccs. Conclusions: Use of the CAPRI applicator in conjunction with 3D volumetric dose planning techniques permits flexibility in dose planning to create anatomically conformable target volumes, which can result in reduced dose to adjacent organs while maintaining dose to the mucosal surface at acceptable levels." @default.
