- W2022635085 abstract "Dementia complicates the management of comorbid illnesses and is associated with increased risk of hospital admissions, but the extent to which Alzheimer's disease (AD) or other dementias might affect the risk of subsequent rehospitalization within 30 days discharge is unknown. For United States hospitals, failure to facilitate successful discharge transitions for Medicare beneficiaries will result in reimbursement penalties for readmissions within 30 days of a hospital stay for several common diagnoses in 2012. The goal of this study was to investigate whether a diagnosis of dementia was an independent predictor of 30-day all-cause rehospitalization of elderly fee for service (FFS) Medicare beneficiaries. This was a retrospective cohort study utilizing Rhode Island Medicare Part A, Part B, and Part D claims data to identify beneficiaries with and without a documented dementia diagnosis who were admitted to Rhode Island hospitals throughout 2009. Odds of 30-day readmission to any U.S. hospital were calculated using conditional logistic regression, controlling for age, gender, race, antipsychotic use, number of comorbidities according to Elixhauser's method, dual eligible status, length of stay of index admission, number of admissions in the previous year, and hospital of index admission. From the cohort of 25,839 patients, 3908 met criteria for diagnosis of AD or other dementia. Nearly 20% of the cohort (N=5,133) was readmitted within 30 days of the index hospitalization. Compared to those without documented cognitive impairment, dementia diagnosis was a predictor of rapid rehospitalization (unadjusted odds ratio, 1.28; 95% CI (1.18, 1.39)). Although attenuated, the association persisted after adjustment for potential confounders, (adjusted odds ratio 1.18; 95% CI (1.08, 1.29)). In general, decreasing unnecessary hospital readmissions is an important strategy for reducing healthcare costs, but avoidance of acute care hospitalizations is particularly important for individuals with pre-existing dementia, because these events are often destabilizing and result in delirium, longer than average hospital stays, and poor post-discharge outcomes. The results of this study suggest that better understanding of the relationship between dementia and effective care transitions is necessary to inform ongoing national quality improvement initiatives aimed at decreasing avoidable readmission rates for hospitalized elderly patients." @default.
