- W2022661437 abstract "Summary The placental transfer of the lipophilic opioids, fentanyl, alfentanil and sufentanil is extremely rapid with peak umbilical venous blood levels occurring within 5 minutes after an intravenous bolus administration to the mother. Systemic absorption from the epidural space is also very rapid and maternal peak plasma concentrations have been measured within 10 to 15 minutes. Absorption from the intrathecal space is only a little slower than absorption from the epidural space. Whether it is fentanyl, alfentanil or sufentanil and whether it is administered intravenously, epidurally or intrathecally, significant placental transfer does occur and thus limits the amount of opioid which can be given safely. We must therefore carefully consider the risks and benefits of the use of opioids and evaluate the studies carefully. Many studies have been performed on relatively small groups of selected patients. The risk of a statistical type II error (risk of overlooking a real difference) is considerable in some of the studies, and the premature or sick neonate may be more sensitive to the depressant effect of opioids than the healthy full term neonate.
