- W2022803055 abstract "Abstract Bile duct tumours are relatively rare in the UK. The most important is cholangiocarcinoma, which occurs most frequently in middle-aged males. Within the UK this tumour has a strong association with primary sclerosing cholangitis and also with abnormalities of the biliary tract, usually cystic in nature. In high incidence countries in the Far East, cholangiocarcinoma is associated with parasitic infections. In the West, the incidence of intrahepatic cholangiocarcinoma is increasing, possibly due to exposure to environmental carcinogens. Most cholangiocarcinomas present late in their natural history, when surgical intervention may be difficult and the mortality is high, despite most tumours being histologically well-differentiated. The role of histology and cytology in the diagnosis and treatment of these tumours is predominantly one of confirmation of diagnosis and stage of disease. Importantly, histology is able to help distinguish cholangiocarcinoma from some of the other lesions on the list of differential diagnoses. Although premalignant changes are recognized, they remain difficult to identify in the diagnostic setting, so effective screening of at-risk populations and treatment of early disease is still something for the future." @default.
