- W2022931070 abstract "Abstract Until recently, hormone replacement therapy (HRT) was the only therapy available which produced substantial changes in bone density, and had been associated with reduced numbers of fractures. There are now a number of options. The bisphosphonates are of comparable efficacy to HRT in the prevention of bone loss and have been shown to halve the risk of vertebral and non-vertebral fractures. The selective oestrogen receptor modulator, raloxifene, also slows postmenopausal bone loss though it is less effective that HRT. It also increases the risk of thromboembolic disease but is associated with a significantly reduced risk of breast cancer. Calcium supplementation significantly retards but does not completely arrest bone loss, but several small controlled studies suggest that it reduces fracture incidence. The maintenance of normal Vitamin D status is important, particularly in the elderly. HRT, the bisphosphonates and raloxifene are all suitable for use in the prevention of postmenopausal bone loss, but the former two are to be preferred in the treatment of established disease. In addition to pharmacological measures, optimising lifestyle and diet play a role, and in the elderly, falls prevention is important." @default.
