- W2022965549 abstract "Kaposi’s Sarcoma (KS) had a low incidence in most Western countries before the advent of the Acquired Imunodeficiency Syndrome (AIDS) epidemic. At that time, in Europe and the United States, the disease occurred mainly in elderly individuals of Jewish and Southern European descent, in whom the male-to-female ratio was approximately 10:1. Clinically, the disease was usually indolent and the prognosis was good. In the 1950s and 1960s, Western doctors working in some parts of Africa reported a high incidence of KS in some parts of Africa, notably countries along the equator. Later, an area of particularly high incidence was defined around eastern Zaire, western Uganda, and adjoining areas. At that time, KS afflicted mainly middle-aged men living in rural areas.. The peculiar epidemiology of KS in Africa stimulated interest into the peculiar features of the disease in Africa; however, early research into factors underlying the epidemiology, clinical diversity, and natural history of the disease raised more questions than answers. Hence, African KS continued to be a medical curiosity and enigma. The epidemic of KS in the United States and Europe in the light of the HIV epidemic has aroused a global scientific interest in the disorder. Studies among Western homosexual men led to a hypothesis that KS was caused by a sexually transmissible agent. This theory subsequently led to the identification of a novel herpes virus, human herpes virus 8 (HHV-8) which is also known as Kaposi’s Sarcoma associated herpes virus (KSAHV). In Africa, as in Western countries, an epidemic of atypical cases of KS led to the recognition of some of the first cases of AIDS. In line with recent developments in the understanding of the etiology of KS globally, HHV-8 or KSAHV has also been detected in patients with African KS. While on a global scale efforts into elucidating the etiology of KS are now being rewarded, the other clinical and social challenges KS poses in Africa continue to challenge physicians caring for patients in Africa. This article provides insights into the dynamics of the KS in Africa over time and highlights changes in its epidemiology, clinical and biological features, and future challenges of the epidemic of KS in some parts of Africa." @default.
