- W2023006802 abstract "ObjectivePreviously published trans-cerebellar diameter (TCD) nomograms have not been prospectively validated. Therefore we prospectively validated our previously published trans-cerebellar diameter (TCD) measurements to determine their accuracy for predicting gestational age (GA).Study DesignPatients between August 2002 and May 2003 with singleton gestations (14-38 weeks) were prospectively enrolled (n = 3072). All subjects had dating confirmed by both menstrual dating and ultrasound performed before 20 weeks. Only one TCD measurement (last scan) was used for each patient for validation of a previously published nomogram from our institution. The observed TCD-derived GA measurement was then compared with the predicted TCD-derived GA (based on our previously published nomogram).ResultsThe agreement between the observed and predicted TCD measurements throughout gestation was high (Pearson correlation coefficient, r = 0.946, P<0.0001). This agreement was superior for prediction of the TCD measurements in the second (r = 0.97, P<0.0001) vs the third (r = 0.89, P<0.001) trimester. Between 17 and 23 weeks' gestation, the predicted GA ranged from 0 to 5 days of observed GA. Between 24 and 31 weeks, the predicted GA was within 2 days, and after 32 weeks, the predicted GA was within 4 days of the observed GA.ConclusionThe accurate prediction of gestational age in this prospective validation study reinforces the reproducibility of TCD measurements. TCD measurement is an accurate predictor of gestational age even in the third trimester. ObjectivePreviously published trans-cerebellar diameter (TCD) nomograms have not been prospectively validated.
