L’atteinte des muscles intrinsèques et la brièveté cutanée sont fréquentes. Le traitement initial est orthopédique et doit être entrepris le plus tôt possible. En cas d’échec de ce traitement médical, le traitement chirurgical doit être adapté aux diverses structures atteintes afin de réanimer les tendons hypoplasiques et de libérer les contractures. Le pouce est atteint dans neuf cas sur dix de doigt à ressaut chez l’enfant. Dans 30 % des cas, l’atteinte est bilatérale. Le terme congénital est discuté car les nombreuses études faites chez le nouveau-né n’ont pas fait la preuve de l’existence de l’atteinte à la naissance. Chez l’enfant il est très rare de trouver un ressaut vrai : il s’agit plutôt d’une déformation fixée en flexion de l’articulation interphalangienne. Le diagnostic est clinique avec un nodule palpable au niveau de la poulie T1. Le traitement conservateur à type d’orthèse peut être proposé jusqu’à l’âge de deux ans. En cas d’échec ou plus tard, le traitement est chirurgical. Congenital clasped thumb in palm corresponds to a spectrum of anomalies leading to a loss of thumb extension and abduction. Intrinsic muscles and skin shortening are not infrequent. Conservative orthopedic treatment should be undergone as soon as possible. When this treatment fails, or when patients are seen late, surgical correction has to be customized according to the involved structures. The trigger thumb is 10 times more frequent than the trigger finger. It is bilateral in 30% of the cases. The term “congenital” remains unclear as several investigations on newborns have not evidenced trigger thumb. In children, it is very rare to find a real trigger and presentation consists in a fixed flexion deformity of the interphalangeal joint. Diagnosis is clinical, with a palpable nodule at the level of T1 pulley. Conservative treatment, consisting in a nocturnal splint, is indicated before the age of two years old.
