- W2023225025 abstract "About 1 in 2000 girls have an imperforate hymen which usually is not recognized before puberty, at which time menstrual blood accumulates in the upper vagina and uterus. Those affected may experience abdominal pain, a distended lower abdomen, and acute urinary retention. This prospective study evaluated 65 women with imperforate hymen, as manifested by cyclical pelvic pain and primary amenorrhea. They were treated by making a central oval incision, inserting a 16 F Foley catheter, and administering estrogen cream locally by application onto hymenal structure for 2 weeks. The object of this procedure is to preserve the integrity of hymenal structure. Patients remained in hospital for 6 hours afterward. Patients had an average age of 14 years at presentation. Hematocolpos was a constant finding. After the procedure, the hymenal orifices remained open and, with 2 exceptions, hymenal structures were intact. In these 2 cases the hymenal orifice reclosed, probably because of inappropriately applying estrogen cream to the hymen. Both these patients had a good outcome after repeat incision and drainage. No patient had bleeding or infection during an average follow-up of 56.5 months. All 28 patients who married during follow-up had bleeding after initial coitus, but none reported severe dyspareunia. Of 19 women who became pregnant, 15 delivered vaginally and 4 had cesarean section. This relatively noninvasive procedure maintains the integrity of the hymenal structure and provides an annular-intact hymenal ring. No complications have emerged." @default.
