Alhough the effects of cardiovascular therapies on morality have been studied intensively, their effects on uality of life have received far less scrutiny. In particlar, coronary revascularization has been examined ore thoroughly for its effect on mortality and irreersible complications than for its effect on functional tatus and quality of life. Fortunately, more clinical trils now measure these important patient-oriented outomes. The sum of evidence from clinical trials of coronary ypass grafting surgery and of coronary angioplasty ompared with medical therapy shows mortality rates re reduced in proportion to the extent of disease. ypass grafting surgery prolongs life in patients with eft main disease or triple-vessel disease, especially in he setting of reduced left ventricular function. Angiolasty does not significantly reduce mortality rates in atients with stable coronary disease, but does so in atients with acute ST-segment elevation myocardial nfarction. The effects of an aggressive approach to oronary angiography and revascularization in acute oronary syndromes has been more controversial. Alhough early randomized trials found no advantage to strategy of routine angiography and aggressive coroary revascularization, recent studies have shown an nvasive approach reduces cardiac events, primarily on-fatal recurrent myocardial infarction. The invasive pproach to acute coronary syndromes costs signifiantly more than a conservative approach, howver, and the value provided by the additional invasive rocedures has not been fully established. A favorable effect on quality of life of the invasive trategy to acute coronary syndromes would provide urther justification for its wider application.
