• W2023338558 abstract "Aortic regurgitation is recognized as a developing clinical problem in the LVAD population. Correlation of AR with renal dysfunction with LVAD has not been investigated. While the impact of LVAD on renal function is described, the correlation to improvement in LV and RV function remains unclear. Our objective was to evaluate the relationship of serum creatinine to LV EF, RV function and AR in the LVAD population. 205 patients underwent implant of LVAD HeartMate II between February 2007 and January 2013 at Mayo Clinic-Rochester. Median age was 63 years, 180 (87%) were male. 130 (61%) were implanted for destination therapy. Echo data, and serum creatinine were retrospectively analyzed preimplant and at 1, 3, 6, 9 and 12 mo. after implant. Degree of AR was divided in groups: none, trivial, mild, moderate, moderate- severe and severe. Degree of RV dysfunction was categorized as normal function/ mild dysfunction, moderate and severe. Correlated data analyses was performed with SAS 9.2. In our population ejection fraction improved from 19.7±9.9mm to 27.8±15.7mm, p<0.001, end diastolic diameter decreased from 67.0±11.1mm to 58.7±11.5mm, p<0.01, end systolic decreased from 61.4±11.8mm to 51.0±12.9mm, p<0.01. These parameters improved continuously at 1,3,6,9, and 12 months. Improvement of LVEF between preimplantation to postimplantation at 1 mo becomes insignificant adding serum creatinine to the model p=0.86. At later time periods improvement in EF remains significant with creatinine added to the model. Further, increased severity of AR correlates with higher creatinine at 1,3,6,9 and 12 months follow up p=0.05. Lastly we observed significant improvement of right ventricle function from preimplantation period to 9th and 12th month (both p<0.001) interestingly without correlation to creatinine. We demonstrate in a large LVAD population, improvement of LVEF over 12 months and improved RV function at 12 months. Importantly we demonstrate that while early post implant creatinine correlates inversely with the improvement in LV function, with longer follow up renal function does not negatively correlate with improved LV EF or RV function. AR does however correlate with increased creatinine. The impact of long term AR on renal outcomes in the LVAD population needs further study." @default.
