- W2023393105 abstract "To the Editor: It was interesting to read the report of Castellaneta et al. (1), which reinforces the utility of upper gastrointestinal endoscopy and ileocolonoscopy in the diagnosis of pediatric inflammatory bowel disease. They have confirmed the value of biopsies from visually affected and unaffected areas. When I reported this approach in 56 pediatric patients in 1991 (2) it was seen, particularly by adult gastroenterology colleagues, as unusual and unnecessary. It is good to see that those earlier recommendations to routinely examine all levels of the gut have been vindicated in this current well designed study. Upper gastrointestinal endoscopy should be performed routinely, not only when there are upper gastrointestinal symptoms or signs. In this recent study upper gastrointestinal endoscopy demonstrated abnormalities in up to 67% of patients and was important in making or changing the diagnosis in uncertain cases. This accords with my original report that upper gastrointestinal tract involvement occurred in 71% of patients and that in 23 of those 56 cases, upper gastrointestinal endoscopy was instrumental in establishing the diagnosis. I concur with their conclusion that all children with suspected inflammatory bowel disease should have upper gastrointestinal endoscopy, including routine biopsies of apparently normal as well as abnormal areas, regardless of symptoms.
