- W2023558227 abstract "Selective decontamination of the digestive tract (SDD) was a revolutionary concept proposed 25 years ago by the intensivist Chris Stoutenbeek and the microbiologist Rick van Saene.1 It has made a profound impression on critical care practice because it is a preventative measure. In contrast, most intensive care therapies are reactive and frequently applied too late in the patient’s illness.2 Part of SDD’s unquestionable success may be that we are actually doing something early which can prevent a cascade of deterioration. The concept is all-encompassing. It re-defines the microbiological reporting of surveillance and diagnostic culture results into 15 potential pathogens, which cause primary and secondary endogenous and exogenous infections. To control primary and secondary endogenous infection, parenteral and enteral antimicrobials are administered. Hygiene and topical antimicrobials are required to control exogenous infections. SDD administration is now the standard of care recommended by the Netherlands Intensive Care Society.3 It is used in many ICUs in continental Europe. However, despite a wealth of level 1A evidence available since 2003, there are more units in the UK which have tried activated protein C and tight glucose control than SDD. Why has it taken the recent publication of one trial 4 to turn heads in the UK, and is it too" @default.
