- W2023598841 abstract "An 82 year-old woman had a cholecystectomy remotely. However, she developed recurrent primary common bile duct (CBD) stones several years later. After unsuccessful endoscopic and medical management, the patient ultimately underwent open common bile duct exploration and choledochoduodenostomy (CDD). However, the patient developed recurrent biliary symptoms about one year after the CDD was created, and was referred to our institution after a bout of acute biliary pancreatitis. At ERCP, the CDD was patent, her bile duct was dilated to 20 mm, and a large periampullary diverticulum was present. Abundant soft yellow material was removed from the CBD after sphincterotomy revision. However, debris rapidly reaccumulated in the CBD, consistent with refractory sump syndrome. It was elected to attempt endoscopic closure of the CDD using a device designed to close cardiac atrial septal defects. After complete CBD clearance at ERCP, the CDD was sized using a biliary extraction balloon, and an appropriately sized atrial septal occluder (ASO) was selected. The CDD was cannulated with a long 0.035” guidewire. An 8 Fr delivery catheter was passed over the wire and across the CDD. The ASO was passed inside the delivery catheter, and the ASO was then deployed across the CDD under endoscopic and fluoroscopic visualization. Sump syndrome complicates a proportion of surgical CDDs. Sump syndrome does not always resolve following biliary sphincterotomy. Endoscopic closure of a CDD using an ASO may be considered in refractory sump syndrome, though this technique remains experimental." @default.
