Cuando requieren actuación, su manejo tradicional ha sido la cirugía, histerectomía o miomectomía, aunque se han propuesto alternativas médicas, pues las intervenciones quirúrgicas se asocian con cierta morbimortalidad y conllevan coste sociosanitario. Dentro de su manejo farmacológico, los análogos de GnRH son los fármacos más conocidos y empleados, aunque sus indicaciones son limitadas por los efectos secundarios asociados a los tratamientos a largo plazo. Su indicación fundamental se basa en el tratamiento prequirúrgico, a histerectomía o miomectomía, y en casos seleccionados de pacientes próximas a la edad de menopausia o que deseen un manejo más conservador. Son capaces de disminuir de manera significativa el volumen uterino, el tamaño del mioma y sus síntomas acompañantes. Su principal desventaja, sin embargo, radica en la reversibilidad de su efecto al suspender el tratamiento, junto con los efectos secundarios asociados al hipoestrogenismo, tales como síntomas climatéricos y pérdida de masa ósea. Las terapias «add-back», que asocian estrógenos en baja dosis al aGnRH, permiten prolongar su uso gracias a la disminución de los efectos secundarios sin afectar a los beneficios. Uterine fibroids are benign tumors that are very common in women and may present significant symptoms in 20%-50% of cases. When they require action, their traditional management has been surgery (hysterectomy or fibroidectomy); however medical alternatives have been proposed, given that surgery is associated with a certain morbidity and mortality and involves healthcare costs. Within the pharmacological management of uterine fibroids, GnRH analogues are the best known and most commonly used drugs, although their indications are limited by the side effects associated with long-term treatment. Their primary indication is based on preoperative treatment (to hysterectomy or fibroidectomy) and in selected cases of patients close to menopause or who want more conservative management. These analogues are able to significantly reduce the uterine volume, the size of the fibroid and their accompanying symptoms.
