D’une part, le patient âgé arrivant aux urgences avec une défaillance d’organe est insuffisamment proposé aux réanimateurs. D’autre part, les réanimateurs ont souvent, comme critère majeur de refus, l’âge chronologique du patient. Les critères d’admission favorables sont une bonne autonomie, un état nutritionnel satisfaisant et l’absence de cancer évolutif. Les critères d’admission défavorables sont un état de gravité où la réanimation est déraisonnable, de lourdes comorbidités, et le risque de dépendance définitive aux techniques de suppléance. Réanimateurs et gériatres doivent se comprendre autour du « grand âge » et des techniques raisonnables inhérentes à la réanimation. Unjustified restriction to intensive care unit (ICU) admission of elderly patients still exists. It can occur in the emergency department where elderly patients may be insufficiently proposed for ICU admission. Moreover, ICU admission criteria for elderly patients rely too often on chronological age only. Admission criteria are self-sufficiency, good nutritional status and no active cancer. Non-admission criteria are severity where resuscitation is unreasonable, heavy co-morbidities and risk of permanent dependence on technology substitution. Intensivists and Geriatricians should discuss what are considered as “Very elderly patients” and reasonable resuscitation." @default.
