Nous présentons un cas d’ostéosarcome chondroblastique mandibulaire traité et suivi durant 14 ans. Le diagnostic et le début du traitement ont été faits 2 mois après les premiers symptômes. Le diagnostic définitif a pu être posé par un examen anatomopathologique de la pièce de résection. Le traitement a nécessité plusieurs interventions pour obtenir des recoupes en tissu sain. Une reconstruction par greffe osseuse ainsi qu’une réhabilitation dentaire ont été effectuées. Ce cas illustre les difficultés rencontrées pour établir un diagnostic correct, réaliser un traitement chirurgical approprié et prendre en charge sur le long terme un ostéosarcome mandibulaire. Pour poser le diagnostic, l’imagerie peut apporter une certaine aide, en recherchant le signe spécifique du triangle de Codman. Cependant seul l’examen anatomopathologique permet un diagnostic fiable. Le traitement de choix est l’exérèse chirurgicale avec des recoupes en tissus sains qui permet d’espérer une survie jusqu’à 80 % à 5 ans. La difficulté essentielle du traitement est liée au délai d’obtention du résultat anatomopathologique. Des interventions répétées sont souvent nécessaires pour obtenir des recoupes en tissu exempt de cellules tumorales. Afin d’éviter des résections multiples et avoir un recul suffisant, une reconstruction différée avec insertion d’une plaque « mainteneur d’espace » temporaire pourrait être une solution. Le pronostic de l’ostéosarcome mandibulaire est meilleur que celui des os longs, mais la chimiothérapie ou la radiothérapie, qui sont très efficaces pour ces derniers, ne changent pas le pronostic de l’ostéosarcome mandibulaire. Osteosarcoma, the second most frequent primary malignant bone tumor, is usually found in long bones: femur, tibia or humerus with only 6 to 7% of cases occurring in the jaws. Patients with osteosarcoma of the mandible usually complain of a swelling, which can be painful or not, accompanied by paresthesia of one of the trigeminal nerve branches in about 20% of cases. We report a case of chondroblastic osteosarcoma of the mandible affecting a 33-year-old woman with 14 years follow-up illustrating the difficulties of a rapid diagnosis. The prognosis of osteosarcoma of the mandible relies on its histological grade and the amount of time elapsing from diagnosis to treatment onset. The treatment of choice is radical surgery providing a 5-year survival rate up to 80%. The definitive surgical treatment is usually performed only after several interventions because operative-room pathology cannot be obtained due to the necessity of decalcification. In order to avoid multiple resections, an interim bridging with a reconstructive plate with a condylar head can be proposed before definitive graft reconstruction. The prognosis of osteosarcoma of the mandible is better than that of long bones. Chemotherapy or radiotherapy, which are very efficient for osteosarcoma in general, do not change the prognosis of osteosarcoma of the mandible." @default.
