Wystąpieniu tych zaburzen towarzyszą zazwyczaj stosowanie niektorych lekow, substancji biologicznych oraz rozne choroby lub stany kliniczne. Badania wlasne wykazaly, ze silne, nawracające bole glowy u niektorych osob uwazanych za immunokompetentne i niezakazone wirusem HIV byly spowodowane reaktywacją nabytej toksoplazmozy mozgowej (CT). Poniewaz bole glowy są jednym z najczestszych objawow IIH, PTC lub AM, mozna przypuszczac, ze wielu tych chorych cierpi z powodu reaktywacji latentnej nabytej/wrodzonej CT. Celem pracy bylo przedstawienie aktualnej wiedzy na temat przyczyn IIH, objawow klinicznych, diagnostyki roznicowej, czynnikow wyzwalających, dotychczasowych zasad leczenia oraz patomechanizmu spowodowanego zaburzeniami immunologicznymi i innymi zmianami laboratoryjnymi, powodującymi w efekcie wystąpienie IIH, PTC lub AM. Dane piśmiennictwa cytowane w pracy zostaly wybrane w celu zilustrowania, ze powyzsze zmiany laboratoryjne mogly w znaczący sposob wplywac na latentną infekcje/intensywnośc stanu zapalnego ośrodkowego ukladu nerwowego (CNS) spowodowaną przez T. gondii. Przykladami czynnikow wyzwalających IIH, PTC, i/lub AM wraz z towarzyszącymi im odpowiednimi nieprawidlowościami laboratoryjnymi są: niesteroidowe leki przeciwzapalne (zwiekszenie produkcji NO, transkrypcji iNOS, wolnych rodnikow tlenowych/azotowych (ROS), TNF-α, IFN-γ, IL-1, IL-2 IL-6; zmniejszenie biodegradacji tryptofanu, indukcja 2,3-dioksygenazy indoleaminy (IDO) przez IFN-γ monocytow, biosynteza prostaglandyn), retinoidy (zmniejszenie syntezy NO, poziomow mRNA oraz bialka iNOS-2, TNF-α w aktywowanym mikrogleju; zwiekszenie IFN-γ, IL-2, czynnika transformującego wzrost-β1 (TGF-β1), IL-12), nitrofurantoina (hiperprodukcja ROS), otylośc (zwiekszenie IL-1β, TNF-α, IL-6, bialka wiązącego GH, leptyny, bialka C-reaktywnego; zmniejszenie NO, IL-2, hormonu wzrostu (GH), insulinopodobnego czynnika wzrostu-I). Wymienione zaburzenia rownowagi miedzy procesami prozapalnymi i przeciwzapalnymi znacznie upośledzaly mechanizmy obronne gospodarza istotne dla kontroli immunologicznej pasozyta, jak produkcja NO, cytokin, ROS, degradacji tryptofanu przez IDO i/lub 2,3-dioksygenaze tryptofanu, ograniczenie dostepności zelaza wewnątrzkomorkowego dla T. gondii, oraz mechanizmow pośredniczonych przez rodzine genu reagującego na IFN-γ, powodując w efekcie reaktywacje latentnej infekcji T. gondii/stanu zapalnego ośrodkowego ukladu nerwowego. Reaktywacja przewleklej toksoplazmozy po leczeniu myszy deksametazonem (supresja reakcji immunologicznej typu TH1), a takze wystąpienie CT po podaniu etanerceptu (hamuje funkcje TNF-α), są zgodne z powyzszymi rozwazaniami. Osoby z IIH, PTC, i/lub AM powinny wiec miec wykonane testy w kierunku zakazenia T. gondii." @default.
