- W2023741427 abstract "Introduction. Heparinase I (Neutralase), a specific enzyme that inactivates heparin, is a possible alternative to protamine [1]. Protamine has been demonstrated to cause severe pulmonary hypertension and systemic hypotension in the dog model, while heparinase I did not [1]. However, there are no regional vascular studies using heparinase I. The effects of heparinase I on the mean arterial pressure and hindlimb perfusion pressure in the male rat [2,3] were studied. Methods. With institutional approval, 5 male Sprague Dawley rats (350-400 g) were anesthetized with intraperitoneal pentobarbital (50 mg/kg). The carotid artery and abdominal aorta were cannulated by cutdown. Hindlimb isolation utilized a semi-closed peristaltic perfusion circuit. Mean arterial pressure (MAP) and hindlimb perfusion pressure (HPP) were recorded. Rats were heparinized (300 U/kg). Heparinase I (IBEX Technologies) (0.625, 1.25, 2.50, 5.00, 10.00 IU/kg) was injected into the hindlimb vascular bed by injection port, and changes in MAP or HPP were recorded. In each rat, saline as a control was injected into the hindlimb circuit. ANOVA with a post-hoc Scheffe' Test was used for statistical analysis, with a p < 0.05 considered significant. Values are mean +/- SEM. Results. Increasing doses of heparinase I caused a significant, but small, decrease in MAP only at the two highest doses (as compared to protamine, which caused a 21% decrease in MAP in an equivalent model). At all doses HPP was significantly less than baseline and saline at one minute. Conclusion. At the clinically applicable doses of heparinase I, (0.625 and 1.25 IU/kg), the decrease in HPP was less than 7% (neither physiologically nor clinically significant, see Graph). Even at the next two higher doses the change was still less than 15%. Heparinase I caused vasodilation in the hindlimb vascular bed and decreased the mean arterial pressure significantly only at supra-clinical doses. Analysis of the mechanism is ongoing.
