- W2023921687 abstract "RATIONALE: Recurrent meningitis requires an immunological workup. We report a patient with three episodes of aseptic meningitis thought secondary to metronidazole. METHODS: case report and literature review. RESULTS: A 42 year old male experienced three episodes of meningism with headache, neck stiffness and photophobia within two hours of taking various forms of Helicobacter pylori eradication therapy containing metronidazole, a proton pump inhibitor and metronidazole. CSF analysis showed raised protein 1648 mg/L, glucose 3.7 mmol/L, white cells 370 (98% polymorphs). No pathogens were identified by microscopy, culture or PCR. Complement analysis including classical and alternative pathway integrity was normal, serum immunoglobulins were normal. The protein pump inhibitor and macrolide components of the eradication therapy were tolerated without ill effect. CONCLUSIONS: Drug-induced aseptic meningitis is recognised with NSAID drugs in particular, but there are very few previous reports of metronidazole-induced aseptic meningitis. Recognition of drug-induced meningitis is important in the immunological work-up of recurrent meningitis. The pathophysiology of this reaction is unknown, but can result in CSF abnormalities suggestive of infection." @default.
