- W2024171623 abstract "従来，冠動脈疾患における心エコー図法の役割は，冠動脈の狭窄または閉塞により引き起こされる壁運動異常あるいは合併症などに対する評価が中心であった．運動負荷あるいは冠動脈拡張薬やカテコールアミンなどの薬物による壁運動の評価が広く行われるようになり，心エコー図による冠動脈疾患の診断精度は向上した．運動負荷心エコー図の冠動脈疾患の診断感度は70‐95％，特異度は75‐95％，ドブタミン負荷心エコー図の感度は75‐90％，特異度は75‐95％と報告されている．また，近年超音波ドプラ法の進歩により，従来心エコー図では困難とされてきた冠動脈血流の描出が可能となった．これにより加速血流描出による冠動脈狭窄の診断あるいは閉塞冠動脈への側副血行路からの血流を捉えることによる冠動脈閉塞の診断，そして薬物負荷を用いた冠血流速予備能の評価が非侵襲的に行えるようになってきている．現在，心エコー図法は解剖学的評価のみならず機能的評価にかかせない検査法として大きな飛躍をとげ，日常診療へ広く応用されてきている．" @default.
