- W2024200601 abstract "Jens Bonde and colleagues (Oct 10, p 1172)1Bonde JPE Ernst E Jensen T et al.Relation between semen quality and fertility: a population-based study of 430 firstpregnancy planners.Lancet. 1998; 352: 1172-1177Summary Full Text Full Text PDF PubMed Scopus (579) Google Scholar report the relation between semen characteristics and the probability of pregnancy and show that the probability of conception increases with rising sperm concentrations up to 40×106/mL. Of the semen variables (semen volume, sperm motility, and morphology), only normal morphology seems to be strongly related to pregnancy probability. On the basis of this finding, the researchers suggest that we think cautiously about the normal limit of 20×106/mL as defined by WHO in the guidelines of the diagnosis of male infertility. However, not sperm concentration but sperm quality determines fertilising capacity of spermatozoa. The results of Bonde's study showing that sperm motility, an important feature of sperm quality, was of limited value in pregnancy prediction, is misleading since the researchers have considered the percentage of sperm motility but not the absolute number of motile sperm. For example, the treatment of hypogonadotropic hypogonadism with gonadotropins allows fertility even if sperm concentration remains lower than normal.2Burris AS Clark RV Vantman DJ Sherins RJ A low sperm concentration does not preclude fertility in men with isolated hypogonadotropic hypogonadism after gonadotropin therapy.Fertil Steril. 1988; 50: 343-347Summary Full Text PDF PubMed Google Scholar, 3Lenzi A Lombardo F Gandini L Alfano P Dondero F Computer assisted sperm motility analysis at the moment of induced pregnancy during gonadotropin treatment for hypogonadotropic hypogonadism.J Endocrinol Invest. 1993; 16: 683-686Crossref PubMed Scopus (20) Google Scholar In our experience 20 patients with idiopathic hypogonadotropic hypogonadism who became sperm positive during gonadotropin therapy conceived when the mean sperm concentration was less than 20×106/mL. In five patients pregnancy occurred when the mean sperm concentration was 1×106/mL. Each case had a population of spermatozoa with very good motility pattern.
