- W2024220929 abstract "Purpose: To achieve useful volumetric modulated arc therapy (VMAT) quality assurance (QA), the dedicated system was developed including the cylindrical phantom allowing dose measurement with rolled-up radiochromic films and the analysis software for dose verification in rolled- out plane of the film. Methods: The acrylic cylindrical body wrapped with radiochromic films (Gafchromic EBT2, ISP, NJ) was inserted into another cylindrical shell of 5 cm thickness. The rolled-up films on cylindrical surface with high resolution enable to detect specific dose error along the arc trajectory of continuously irradiated beams in VMAT. The developed software facilitates dose evaluation in rolled-up/-out plane of the film; the corresponding calculation doses on the points where the rolled-up film was placed were reconstructed into the rectangular dose matrix. The reconstructed one was compared with the scanned values of irradiated rolled-out films. Dose disagreement by the gamma index (3 mm / 3%) in rolled-out plane was also distinguished on cylindrical coordinates of ERGO++ (version 1.7.6, Elekta Ltd., Crawley, UK). Detected dose error was analyzed according to the gantry angle and presented with principal organs to differentiate the clinically significant dosimetric effect. Results: As the VMAT QA with the developed system was applied for pelvic metastasis, prostate, and nasopharynx cancers, the nearly identical pattern of dose distributions were reconstructed. Local region not satisfied with the gamma criteria was differentiated in certain gantry angle and peripheral region of the fields. Primary gantry angle causing dosimetric error could be easily caught out in cylindrical and anatomical coordinates. Conclusions: The developed VMAT QA system provides the practical and reliable QA method to detect dosimetric error according to the gantry rotation. Film dosimetry in rolled-up/-out plane leads to the dose verification in subspace of the 3- dimensional dose verification. The system can be employed as an effective tool to the patient-specific VMAT QA." @default.
