- W2024227144 abstract "To examine the association between weight gain and adverse pregnancy outcomes in twin pregnancies. A historical cohort 169 twin pregnancies cared for in a single maternal-fetal medicine practice from 2005-2008 were identified. In order to account for the expected positive association between overall weight gain and gestational age at delivery, the weight gain per week was used instead of total weight gain. Maternal weights were obtained from direct measurement. Pearson correlation, chi square, and Student′s t-test were used when appropriate. The overall mean weight gain was 1.06 +/− 0.42 lbs/week. Weight gain per week was significantly positively associated with gestational age at delivery, birthweight of the larger twin, and birthweight of the smaller twin. We compared the 87 patients with weight gain of at least 1lb/week to the 82 patients with less than 1 lb/week. The two groups had similar baseline characteristics (age, weight, IVF, multifetal reduction, chorionicity, and cervical length at 22-26 weeks), however, the group with weight gain of at least 1 lb/wk had a significantly later GA at delivery (36.13 +/− 2.13 vs. 35.25 +/− 3.13, p =0.033) , lower risk of preterm birth <32 weeks (4.6% vs. 14.6%, p=0.026), greater birthweight of the larger (2648 +/− 501g vs. 2423 +/− 608g, p=0.009) and smaller twin (2317 +/− 533g vs. 2137 +/− 594g, p=0.039), and a higher likelihood of both twins being >2500g (41.4% vs. 26.8%, p=0.046). The group with weight gain of at least 1 lb/week did not have an increased risk of hypertension (14.8% vs. 14.6%, p=0.955) or gestational diabetes (6.9% vs. 12.2%, p=0.240). In twin pregnancies, increased weight gain is associated with prolonged gestation and increased birthweight, without an increased risk of diabetes or hypertension. Consideration should be given to recommend weight gain of at least one pound per week in twin pregnancies as a preventative measure against preterm birth and growth restriction." @default.
