- W2024334768 abstract "Background/Aim: Autofluorescence imaging (AFI) is a novel procedure to detect autofluorescence emitted from intestinal tissues and applies for the alterations of the fluorescence intensity to diagnose the intestinal lesions. Recently, videoendoscopy with AFI was considerably developed and used for detecting early stages of neoplasia in patients with Barrett's esophagus. Furthermore, recent works have also presented of AFI videocolonoscopy to detect colorectal lesions. The aim of the present study was to evaluate the efficacy of AFI videocolonoscopy for the histological diagnosis of colorectal lesions. Materials and Methods: From December 2006 to July 2008, we retrospectively studied 430 colorectal lesions, 87 with hyperplastic polyp, 275 with adenoma, and 68 with cancer. Cancers were composed of 22 intramucosal or slightly submucosal invasive cancers, 15 massively submucosal invasive cancers, and 31 advanced cancers. Histology was assessed by the biopsy specimen, endoscopic mucosal resection, or surgical resection. AFI images were classified subjectively into 4 categories: G; green, GP; green with purple spots, PG; purple with green spots, and P; purple. Furthermore, we investigated to quantify the AFI image by using a special software which can calculate the fluorescence/reflex ratio (Fluorescence Index, FI). Results: 1) When considering to be neoplasm, i.e. adenoma and cancer, if the lesions exhibited P or PG on AFI images, the diagnostic value was 85.8% for accuracy, 88.7% for sensitivity, 52.9% for specificity, and 94.2% for positive predictive value. With the lesions of P image by AFI, however, the incidence of cancer was unexpectedly low (44.0%). 2) The FI was 1.152 ± 0.076 in hyperplastic polyp, 0.801 ± 0.027 in adenoma, and 0.653 ± 0.070 in cancer. The FI of cancer was significantly higher in comparison to that of hyperplastic polyp and adenoma (p<0.001). However, the FI was not associated with the depth of cancer invasion. Conclusions: Our results indicate that the quantitative analysis (FI) of AFI is possibly useful to make the histological diagnosis of colorectal lesions, although the optical and subjective AFI images do not reflect the histology. AFI videocolonoscopy may be a valuable modality for the histological distinction in colorectal lesions." @default.
