- W2024373567 abstract "The past two decades have heralded the advent of evidence-based medicine in the field of back pain with a focus on high-quality randomized clinical trials (RCTs), systematic reviews, and meta-analyses to determine the efficacy behind various treatments. The assumption generally held is that if a treatment has been shown to be efficacious, it should be made available to all patients likely to benefit, irrespective of the cost. Important principles regarding the evaluation of cost-based studies on back pain are reviewed in an article by William Johnson appearing in this issue of The Spine Journal. We can expect many papers of this type to enter the spine literature, and we cannot ignore the message that is implicit in these articles. Economic principles are increasingly important as many RCTs are now demonstrating that although treatments may be beneficial when compared with placebo, the long-term outcomes of many different treatments look similar. Even when a treatment approach is shown to have statistical benefit over another treatment, it is not uncommon to note that the difference often is very small and may not be clinically meaningful. As the cost of treating patients with back pain has escalated, there is a wide perception that there has been little impact on the degree of disability associated with these symptoms." @default.
