- W2024617797 abstract "Goal: The diagnosis of early chronic pancreatitis can be difficult to make. Early fibrotic changes in the pancreas may be below the limits of detection of current imaging modalities. We present an analysis of all subjects seen in our institution over the last decade who had unexplained chronic abdominal pain and normal abdominal imaging. Each of these subjects underwent a secretin stimulation test (SST) as part of the evaluation of their symptoms. The goal of this study was to describe the secretin test characteristics of this unique patient sample. Methods: We evaluated all subjects who underwent an SST at our medical center during the period from Jan 1995-December 2005. Ninety-five patients were included in the study. Individuals with measured bicarbonate of 70 meq/L or less were considered as having definitive chronic pancreatitis (CP). A measured bicarbonate of 75 meq/L or greater was considered as not having chronic pancreatitis (CP). Individuals with a bicarbonate concentration of 71-74 meq/L were interpreted as indeterminate. Cumulative frequency analyses were conducted and the distribution of the individual patient diagnostic groups was analyzed. Results: Twenty four (25%) subjects had definitive pancreatitis. Sixty three (66%) subjects had no disease and seven (9%) subjects were indeterminate. Sixty percent of the subjects were female and 34% were male. All of the indeterminate subjects were female. Thirty minutes or less was the time to peak bicarbonate concentration for the majority of subjects with CP. The same was true for a subject without CP. Forty five minutes was the time to peak bicarbonate in the majority of indeterminate subjects. One hundred percent of subjects without CP were diagnosed by 45 minutes, 97.6% of subjects with CP and 70% of indeterminates were definitively diagnosed by 45 minutes. There was no significant difference between outcomes in males or females. Conclusions: For subjects suspected of having CP with chronic abdominal pain and normal abdominal imaging approximately, 25% will have CP. Greater than 98% of these individuals would be correctly diagnosed by SST which lasted only 45 minutes vs. 60 minutes. The time to peak bicarbonate concentration did not differ significantly among the groups. We conclude that secretin SST is useful in the diagnosis of CP in subjects with chronic abdominal pain and CP." @default.
