- W2024962799 abstract "胸骨圧迫式心マッサージの他,active compression-decompressionCPR (ACD-CPR)および開胸心マッサージ,PCPS中の脳血流と脳酸素代謝について持続内頸静脈酸素飽和度(SjO2)を測定することで,脳蘇生の視点からこれらの各種における心肺蘇生法について再評価した。当施設にて入院加療した症例の中で心停止直前にSjO2が測定された30例で検討した。年齢は14～81歳,平均56.3±2.1歳で,男性24例,女性6例であった。いずれの症例も最終的には不可逆的な心停止に至り,全例が死亡した。SjO2測定中の動脈血中酸素飽和度(SaO2)はいずれも95%以上を呈した。SaO2値が95～100%の正常値を示す場合は,SjO2値はFickの原理から60～80%を呈することが知られている。胸骨圧迫式心マッサージと人工呼吸を5対1で行う方法は16例を行い,その平均SjO2値は69.1±8.6%で平均最高動脈圧は62.2±6.1mmHgであった。ACD-CPRは5例に施行したが,平均SjO2値は74.8±14.1%で,平均最高動脈圧は64.1±6.2mmHgであった。さらに開胸心マッサージは4例に行い,平均SjO2値は75.1±10.3%と今回の検討では最も高値を呈した。平均最高動脈圧は70.0±18.9mmHgであった。PCPSは5例に行い,平均SjO2値は70.0±10.8%,平均最高動脈圧は73.5±21.8mmHgと血圧は最も高値を呈した。すべての症例でSjO2値は60～80%を示した。各種心肺蘇生法における脳血流と脳酸素代謝を比較検討した結果,いずれの方法でも脳酸素代謝に見合った脳血流が確保されていた。すなわち,正しい心肺脳蘇生法を施行すれば脳循環は維持されることが明らかとなった。" @default.
