We introduce the operative procedure and clinical outcomes of the Seoul technique using the GreenlightTM HPS 120W laser. METHODS The Seoul technique includes a planned vaporization-resection technique for PVP, not vaporization only. It is vaporizing the prostate along the outlined margins and retrieving the resulting wedge-shaped prostatic tissue. This operating method consists of following three steps. The first step is the wedge resection of the median lobe. We dig into the prostate along both lateral margins of the median lobe. Then we can get the wedge-shaped tissue of the median lobe. The second step is the wedge resection of both lateral lobes. Before the resection of the lateral lobe, we make two semi-circular lines on the distal part of both lateral lobes at the level of the verumontanum with a constant depth, which is the distal margin of the TUR-like cavity. The external sphincter can be saved with this procedure. We make a line on the lateral lobe along the length of the urethra starting at the concave point at bladder neck and ending at the level of the verumontanum. If the lateral lobe is too big, it may be divided into several pieces to allow evacuation without morcellation. The third step is making the smooth surface of the TUR-like cavity. After removal of the lobes, there may be some protruding or remnant tissues which may interfere with the complete cavity. So we must flatten the rough surface and make the complete cavity by additional vaporization. A total of 140 patients that underwent PVP were included in this retrospective study. 68 patients were treated with the vaporization-only technique and 72 patients with the Seoul technique. The efficacy and safety was evaluated by the International Prostate Symptom Score (IPSS), quality of life score(QoL), peak flow rate(Qmax), postvoided residual (PVR) and complications at 0.5, 1, 3, 6 months following treatment. RESULTS In both groups, IPSS, QoL score, Qmax, and PVR were improved significantly than baseline beginning from 0.5 to 6 months after the PVP. Compared to the vaporization only group, we could save more lasing time and laser energy in Seoul technique group. CONCLUSIONS The Seoul technique for the PVP using GreenlightTM HPS 120W laser showed good short-term clinical outcomes and safety for the treatment of BPH patients. This technique has the advantages of saving more lasing time and laser energy, and getting prostatic tissue.
