• W2025118511 abstract "To investigate whether internal echo histogram with three-dimensional ultrasonography can contribute to the differentiation of malignant ovarian masses from benign masses, and of histologic types of ovarian masses. We performed three-dimensional ultrasonographic examination before surgical operation for ovarian masses. The images of ovarian masses were reconstructed to internal echo histograms. We compared the results of histograms using mean and peak gray index (scale 0–255) between malignant and benign epithelial ovarian tumors, and also among different histologic types (epithelial tumor/endometriosis/mature cystic teratoma). 62 women were included; eight had a malignant epithelial tumor, six had a borderline epithelial tumor, 17 had a benign epithelial tumor, 15 had an endometriotic cyst, and 16 had a mature cystic teratoma. The mean gray index of malignant and/or borderline epithelial tumors was higher than that of benign epithelial tumors (36.9 ± 16.2 vs. 50.7 ± 21.8, P = 0.040). ROC curve analysis showed an area under the curve (AUC) of 0.72 (95% CI, 0.53–0.86) and a sensitivity of 50.0%, specificity of 94.1%. The peak gray index of malignant and/or borderline epithelial tumors was also higher than that of benign epithelial tumors (12.5 ± 14.8 vs. 24.3 ± 22.2, P = 0.037). Sensitivity and specificity of peak gray index were 92.9% and 47.1%, respectively, with an AUC of 0.72 (95% CI, 0.53–0.86). The mean and peak gray index for benign epithelial tumors were lower than those of endometriotic cysts and mature cystic teratoma (36.9 ± 16.2 vs. 52.7 ± 31.4 vs. 66.0 ± 37.5, P = 0.04; 12.5 ± 14.8 vs. 38.6 ± 29.5 vs. 33.1 ± 26.0, P = 0.002, respectively). Also, the patterns of histogram were different according to homogeneity of internal component of ovarian masses. The quantitative and qualitative differences of internal echo histograms of ovarian masses might offer additional differentiation of malignant ovarian masses from benign masses, and of histologic types." @default.
